Consultant-led aesthetics

Facial assessment with Consultant Plastic Surgeon Dr Mobin

Dermal filler treatment at Santi is planned around anatomy, proportion and long-term balance. Dr Mobin's surgical background brings a detailed understanding of facial structure, ageing and tissue support to the consultation process.

The aim is not to fill every line, but to decide whether filler is appropriate at all — and, if so, where the smallest effective amount can create the most natural result.

What fillers can improve

Restore, refine and support

Restore volume

Cheeks, temples and selected under-eye transitions where age-related volume loss is the main issue.

Refine contours

Chin, jawline and profile balance using structural support rather than indiscriminate filling.

Soften shadows & folds

Nasolabial and marionette folds may be softened directly or indirectly through mid-face support.

Is filler the right treatment?

Volume loss, skin quality or muscle movement?

Not every line or ageing concern needs filler. One of the most important parts of consultation is deciding which tissue or mechanism is actually creating the concern.

Main concernUsually considered firstWhy
Loss of facial volume or structural support Dermal filler HA filler can replace or redistribute volume and support contour.
Dryness, crepiness or skin quality Skin boosters Skin-quality treatments focus more on hydration and tissue quality than shape.
General hydration / laxity without volume loss Profhilo Useful where the aim is skin quality rather than structural augmentation.
Thin, crepey under-eye skin Polynucleotides May be preferable where tissue quality rather than volume deficit is the main issue.
Dynamic forehead, frown or crow's-feet lines Wrinkle-relaxing consultation Lines caused primarily by muscle movement require a different treatment strategy.
Texture, pigment or significant skin damage CO₂ / skin treatment Surface and collagen-remodelling concerns often need a skin-focused treatment instead.

What happens at Santi

1

Full-face assessment

We look at facial proportions, movement, volume loss, skin quality, previous filler and your goals.

2

Conservative treatment plan

Product, volume and placement are chosen for tissue depth and anatomy. Numbing is available for comfort.

3

Review after settling

Swelling and integration are allowed to settle before deciding whether any further treatment is genuinely needed.

Longevity

How long do dermal fillers last?

Longevity varies substantially by area, product, movement, dose and individual metabolism. Area-specific ranges are more useful than one blanket figure.

AreaTypical guideWhy it varies
LipsApproximately 6–9 monthsHigh movement and relatively soft filler products.
Cheeks / chinApproximately 9–18 monthsDeeper structural placement and firmer HA gels may persist longer.
Tear troughApproximately 9–12 monthsLow movement, but treatment is only suitable in carefully selected patients.
Nasolabial / marionetteOften around 6–12 monthsDepends on movement, product and whether mid-face support is also addressed.
Safety first

Why anatomy matters with dermal fillers

Dermal filler is an injectable medical-aesthetic procedure. Common effects such as swelling, tenderness and bruising are usually temporary, but rare complications can be serious. Treatment therefore requires detailed anatomical knowledge and an urgent-management protocol.

Common temporary effects

  • Swelling and tenderness
  • Bruising or small injection marks
  • Temporary firmness or asymmetry while swelling settles
  • Short-lived redness

Important risks discussed at consultation

  • Infection
  • Persistent lumps or asymmetry
  • Delayed inflammatory reactions
  • Vascular compromise / occlusion
  • Rare tissue damage or visual complications
HA filler reversal: hyaluronidase is available when clinically indicated. This does not remove the need for careful prevention, early recognition and urgent management of complications.

Tips for when getting lip filler

Our philosophy

Natural-looking filler is often about what we choose not to fill

Every face has different proportions, skeletal support, fat distribution and movement. A one-size-fits-all approach can create unnecessary heaviness and gradually distort features. We therefore plan treatment around the whole face, not the popularity of a particular treatment area.

Lips

Lip enhancement can improve hydration, border definition, symmetry and proportion. We favour conservative volumes and soft gels appropriate to the movement of the lips.

Cheeks and mid-face

Loss of deep mid-face support can contribute to under-eye shadowing and more prominent nasolabial folds. In suitable patients, treating the structural cause can produce a more natural result than directly filling every line.

Tear troughs

Tear-trough filler is not suitable for everyone. Thin skin, oedema, malar bags and anatomical variation can make direct filler a poor choice. Mid-face support or skin-quality treatment may sometimes be preferable.

Chin and jawline

Structural filler can improve selected profile imbalances and lower-face proportions. Larger-volume treatment should still remain proportion-led rather than aiming for a generic angular jaw.

Non-surgical rhinoplasty

Nose filler is an advanced indication because of vascular anatomy. It requires careful patient selection, conservative technique and a clear understanding of the limits of non-surgical correction.

Questions

Dermal filler FAQs

Which filler brands do you use?

We select from premium CE-marked hyaluronic acid ranges according to anatomy, tissue depth and treatment goal. The specific product is discussed during consultation.

Will dermal filler look natural?

That is the aim. We prioritise proportion, conservative volumes and staged treatment rather than maximal correction in one session.

How long does filler last?

It varies by area and product. Lips often last approximately 6–9 months, while structural cheek or chin filler may persist around 9–18 months. Individual metabolism and movement matter.

Am I suitable for tear-trough filler?

Only after careful assessment. In many patients, cheek support or a skin-quality treatment such as polynucleotides may be more appropriate than direct tear-trough filler.

Can HA filler be dissolved?

Hyaluronic-acid filler can be dissolved using hyaluronidase when clinically indicated. Dissolving filler also has risks and requires assessment.

What are the risks?

Bruising, swelling, tenderness, asymmetry and lumps can occur. Rare but serious complications include vascular occlusion and tissue or visual injury, which is why anatomy, early recognition and urgent management protocols matter.

Can I have fillers during pregnancy or breastfeeding?

We do not perform elective dermal filler treatment during pregnancy or breastfeeding.

What if my lines are mainly caused by movement?

Filler may not be the right treatment. Dynamic lines may be better assessed through a wrinkle-relaxing consultation, with sequencing discussed where several treatments are appropriate.
